Form W-4, Employee's Withholding Allowance Certificate Instructions

Form W-4 authorizes the OSU Central Payroll Office to withhold Federal and Oregon taxes based on your filing status and withholding allowances you claim on the form. By default, the number of allowances you claim on Form W-4 for Federal is also used for the State of Oregon.

If you wish to have a different withholding allowance for Oregon, please fill out a separate Form W-4 and print "For Oregon Only" at the bottom of the W-4 certificate. A Form W-4 remains in effect until you send another one to the OSU Central Payroll Office. If you claim exempt status, you must submit a new form by February 15th each year to continue this status.

The year indicated on the form must match the year of the filing date.  The W-4 form must not be altered or appear to be altered.  Scratching out a mistake invalidates the form.  If an error is made you will need to complete a new form. 

Completed forms must be received by the OSU Central Payroll Office by the 15th of the current month.
