Employment Category Change Waiver - Instructions

Use this waiver when a classified employee requests or voluntarily accepts a change in appointment percent or appointment type (employment category) and is waiving access to layoff rights, per Article 48 in the SEIU collective bargaining agreement. This waiver is an attachment that may accompany a classified job change form.
