EMPLOYEE LEAVE / REVISED LEAVE WITHOUT PAY REQUEST - INSTRUCTIONS

Use this task to revise the leave begin or leave end date on a previously submitted Leave Form (Place Job on Leave).

To complete this task, enter the employee’s University ID number and the transaction number from the original form. Transaction numbers are located on the right-hand side of the “Approvals / Signatures” title bar on the original form.

Required authorizations

If leave without pay is at employee's request AND the employee is available to sign, an employee signature is required.

Items to attach to this form

Leaves at employee's request: a copy of the Employee Leave Request form or letter.

Where to send completed form

Please send to the human resources staff in your Business Center.

Unclassified Employees are not required to exhaust all leave balances before leave without pay is potentially available. For more information see the OSU Faculty Requested Leave Without Pay Policy and Procedures.

Classified Employees:

Leave without pay (unrelated to educational leave, military leave or Peace Corps leave) may be requested as follows:

Leave unrelated to reasons noted above or military leave: all leave balances must be exhausted except sick leave before leave without pay is potentially available.
