Academic departments, research centers, and training grant principal investigators may appoint postdoctoral fellows if they meet eligibility requirements as outlined by the OSU Graduate School. Postdoctoral fellowships are non-service/non-payroll appointments. The purpose of this appointment is to identify postdoctoral fellows who are eligible for health insurance coverage, access to travel insurance, and workers’ compensation as applicable under workers’ compensation law.
Before initiating a postdoctoral fellowship appointment, please review the existing OSU employment status of the individual you wish to appoint. No other appointment type may be held concurrently, other than another postdoctoral fellow position. It may become necessary to terminate another appointment if it overlaps the proposed postdoctoral fellowship appointment period.
All sections of this form are completed by the appointing department or training grant principal investigator.
A postdoctoral fellow Banner position number is needed to complete this section. If your college/unit does not have one established for your orgn code, email Jennifer Short in the Office of Human Resources at email@example.com and provide your orgn code and index. Postdoctoral fellow position numbers begin with C94xxx. The same pooled position number is used for all postdoctoral fellows in the same orgn code.
Appointments should be made for one contract year only. Multi-year fellowships are to be renewed annually.
If it becomes necessary to end a postdoctoral fellowship appointment before the original appointment end date transpires, please submit an appointment form, indicating the new end date and reason for ending the appointment.
Complete this section for the initial appointment only. All fields are required. The social security number is used for IRS reporting.
This section initiates disbursement of stipend funds. List both the monthly and total annual stipend amounts.
Stipends issued through the OSU Office of Post Award Administration will be issued to the fellow’s current vendor payment address. The fellow CANNOT receive a stipend until a current VP address exists in Banner. If there is no current VP address, complete and submit a vendor maintenance form to have one created.
